The film’s events immediately follow those of The Matrix Reloaded and assume familiarity therewith.

Bane/Smith and Neo are both in an unconscious state. Both men appear to be asleep, but neural patterns show a Matrix similarity in Bane and true Matrix patterns in Neo. Morpheus, dispirited after the destruction of the Nebuchadnezzar and discovering the true nature of the Prophecy at the end of the last film, starts a search for Neo within the Matrix. Neo is in fact trapped in a subway station named “Mobil Avenue,” a transition zone between the Matrix and the Source (the Machines’ mainframe). At this station, Neo meets a ‘family’ of programs, including a girl named Sati, whose father tells Neo that Mobil Avenue is controlled by a program called the Trainman who is an exile loyal only to the Merovingian. When Neo tries to board the train with the family, the Trainman refuses to let him aboard.

Seraph contacts Morpheus on behalf of the Oracle, who informs Morpheus and Trinity of Neo’s confinement in Mobil Avenue. Seraph, Morpheus, and Trinity pursue the Trainman to secure Neo’s release, but he escapes. The trio enter Club Hel to confront the Merovingian for Neo’s freedom. The Merovingian demands “the eyes of the Oracle” in exchange for Neo’s release. Trinity loses her patience and provokes a Mexican standoff, forcing the Merovingian to release Neo.

The Architect and the Oracle agree to unplug all humans who want to be freed. The closing shot of the film depicts a new dawn created by Sati on the world of the Matrix, which is now free of its characteristic green tint.

Six months after the events of the first movie, Neo wakes from a nightmare wherein Trinity had been shot by an Agent of the Matrix while falling from a building. Morpheus receives a message from captain Niobe (Jada Pinkett Smith) of the Logos calling an emergency meeting of all Zion’s hovercraft fleet. She has successfully recovered the information left by Captain Thaddeus (in the Animatrix short film Final Flight of the Osiris, recovered in the video game Enter the Matrix): 250,000 Sentinels are tunneling towards the underground city of Zion and will reach it in 72 hours. Commander Locke, the ranking military officer of Zion, orders all ships and their crews, including Neo, Trinity, Morpheus, and their new operator Link, to return to Zion to prepare for the onslaught of the machines. Morpheus asks a ship to remain behind to contact the Oracle, in defiance of Commander Locke’s orders. The Caduceus receives a message from the Oracle, and the Nebuchadnezzar, also in defiance of Locke’s order, ventures out to allow Neo to contact her. Meanwhile, one of the Caduceus crew members, Bane, encounters Agent Smith, who takes over Bane’s body. Smith then leaves the Matrix via the nearby hard line and into Bane’s body.

After conversing, the Architect gives Neo a choice of two exits from the room: one door will lead to the rebuilding of Zion, the other door to Trinity’s rescue, causing the downfall of humanity. Neo hastens from the building in an attempt to save Trinity, who is shot by an Agent as they fall from a building and is caught by Neo before she hits the ground. Refusing to accept her impending death, Neo removes the bullet lodged in her chest and manages to revive her by stimulating her heart. In the Nebuchadnezzar, Neo explains his findings to the crew, revealing that the prophecy of the One is false and that Zion will be destroyed in twenty-four hours. Sentinels approach and destroy the ship, whereupon Neo saves his friends by using an unexpected ability to overload and disable the Sentinels’ program with an EMP-like shockwave. The exertion inherent in this effort causes Neo to fall suddenly unconscious. The crew is picked up by the hovercraft Mjolnir , whose crew explains that they were to lead a pre-emptive attack on the army of Sentinels advancing on Zion. The strategy was to use the ship’s EMPs and stay out of each other’s range; but an EMP was detonated by the Caduceus before the ships could move into position. Therefore multiple ships were left without power, with only a handful of survivors escaping, the Sentinels having destroyed any ship that could not get away. Only one survivor was found aboard the Caduceus: Bane.

Computer programmer Thomas A. Anderson leads a secret life as a hacker under the alias “Neo”, and wishes to learn the answer to the question “What is the Matrix?”. Cryptic messages appearing on his computer monitor and encounters with several sinister agents lead him to a group led by the mysterious Morpheus, a man who offers him the chance to learn the truth about the Matrix. Neo accepts by swallowing an offered red pill, and abruptly finds himself naked in a liquid-filled pod, his body connected by wires to a vast mechanical tower covered with identical pods. The connections are severed, and he is rescued by Morpheus and taken aboard his hovercraft, the Nebuchadnezzar. Neo’s neglected physical body is restored, and Morpheus explains the situation.

Morpheus informs Neo that the year is not 1999, but estimated to be closer to 2199, and that humanity is fighting a war against intelligent machines created in the early 21st century. The sky is covered by thick black clouds created by the humans in an attempt to cut off the machines’ supply of solar power. The machines responded by using human beings as their energy source, later growing countless people in pods and harvesting their bioelectrical energy and body heat. The world which Neo has inhabited since birth is the Matrix, an illusory simulated reality construct of the world as it was in 1999 developed by the machines to keep the human population docile in their captivity. Morpheus and his crew are a group of free humans who “unplug” others from the Matrix and recruit them to their resistance against the machines. Within the Matrix, they are able to use their understanding of its nature to bend the laws of physics within the simulation, giving them superhuman abilities. Morpheus believes that Neo is “the One”, a man prophesied to end the war through his limitless control over the Matrix.

As Neo runs through the city toward another telephone exit, he is pursued by the Agents while “Sentinel” machines converge on the Nebuchadnezzar’s position in the real world. Neo reaches an exit, but he is ambushed by Agent Smith and shot dead. In the real world, Trinity whispers to Neo that she was told by the Oracle that she would fall in love with “the One”, implying that this is Neo. She refuses to accept his death and kisses him. Neo’s heart beats again, and within the Matrix, Neo revives; the Agents shoot at him, but he raises his palm and stops their bullets in mid-air. Neo is able to perceive the Matrix as the streaming lines of green code it really is. Agent Smith makes a final attempt to kill him, but his punches are effortlessly blocked, and Neo destroys him. The other two Agents flee, and Neo returns to the real world in time for the ship’s EMP weapon to destroy the Sentinels that had already breached the craft’s hull. A short epilogue shows Neo back in the Matrix, making a telephone call promising that he will demonstrate to the people imprisoned in the Matrix that “anything is possible”. He hangs up the phone and flies into the sky.

Note: The story is explained here in its chronological order, rather than the way it unfolds in the film.

Leonard Shelby (Guy Pearce) wakes up in an anonymous hotel room oblivious to his state. He has a phone conversation with an unknown caller in which he relates the story of Sammy Jankis (Stephen Tobolowsky), a man who suffered from anterograde amnesia, which prevented him from forming new memories. Leonard was previously an insurance fraud investigator assigned to determine if Sammy’s condition could be covered under his insurance policy. If his condition qualified as a physical injury that prevented him from carrying out his job, he and his wife (Harriet Sansom Harris) would be covered.

During his research, Leonard discovered that anterograde amnesia sufferers are still able to condition themselves to perform certain tasks. After Sammy repeatedly failed a conditioning test, Leonard concluded that Sammy’s memory loss was psychological, not physical, and the insurance claim was denied on grounds that Sammy was not covered for mental illnesses. Under the impression he could be cured if this was true, Sammy’s wife came back to the insurance agency, asking Leonard if he believed that Sammy was faking his short-term memory loss. Believing she wanted some kind of answer, Leonard diplomatically restated the insurer’s position: he believed Sammy was physically capable of creating new memories. Sammy’s wife, a diabetic, tried to confirm her belief that Sammy could make new memories by testing him, asking him repeatedly to give her an injection of insulin, every 20 minutes. After several doses, he unknowingly administered an overdose sending his wife into a coma. Sammy was later confined to a mental institution, incapable of remembering her death.

When Natalie learns Dodd is gone, she has a friend trace John G.’s license plate number from Leonard’s tattoo. She gives him a copy of the man’s driver’s license, and Leonard matches the ID to his photo of Teddy, whose real name is John Edward Gammel – “John G.”. Leonard’s plan for himself is complete, even though he’s forgotten that there was any plan at all: he concludes that Teddy is the man who raped and killed his wife. He takes Teddy to the abandoned building where he killed Jimmy Grantz only a few days before and shoots him in the head. Ignorant of the events that led him to this moment, Leonard takes one last picture of the murder scene, a memento of his quest for revenge.
